CUSD families, please complete a Household Income Data form to help ensure continued state funding for programs and services that directly support positive student outcomes. Household Income Data forms have been sent home (see below for an example), or may be completed online using these links:

The state requires us to collect household income data every four years to determine our eligibility for funding used to support:

  • School safety personnel

  • Chromebooks

  • Social-emotional and mental health services

  • School nurses and health clerks

  • College and career readiness

  • Visual and performing arts

  • After-school and summer programs

  • Free Advanced Placement (AP) testing

  • Free sports physicals

  • Family engagement programs

  • Breakfast and lunch for all students

  • And more!

Information for each of your children attending CUSD schools should be entered into a single form per household. The information you provide is confidential and protected by relevant data privacy laws.
